Change Personal Details
Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you.
Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).
Out of Area patients
Should a patient move to an address that falls outside of the practice catchment area, they will be given 30 days to register with a new practice closer to their new address. To find their nearest GP click on this link: Find a GP – NHS (www.nhs.uk).
During the 30 days they will not be entitled to a visit by The Mill Medical Practice.
Once the 30 day period has lapsed, the patient will be automatically deducted from our patient list.
